Upon request, we will also tell patients how we use their personal information. As well as notifying the disclosure of their information to whomsoever, if applicable.
 
We will make the requested information available within 30 business days. Provide a written notice of an extension where further time would need to fulfill the request.
 
Minimum fees may be billed for providing access to personal information. If a fee needs to be utilized, we will inform the patients of the cost. A request further direction from the patients on whether we should proceed with the request, or cancel altogether.
